Mr. X is 42 years old man of polish origin builder living with young family of 3 childrens in a 3 bedroom terraced house. His past medical history include intermittent chronic low back pain always got better with few days rest and over the counter medications. He also stated to having no allergies and has never taken any recreational drugs. He is non -smoker and non- drinker. Mr X stated that he has not bough any other over the counter medications including any herbal remedies except paracetamol and ibuprofen. Five days ago Mr. X developed some discomfort in his low back after lifting 25 kilograms cement bag at work, which he reported got worse after the night sleep. He could not able to get out of the bed on next day morning. He took paracetamol 500 mgs four times a day with no effect so after second day he bought ibuprofen 250mg over the counter and has been taking last four days. He reported his pain levels were same and not been able to return to work for the last 5 days. He was using hot water bottle before go to bed and his wife massaged few times which actually made his pain worse. IP- independent prescriberSP- supplementary prescriber.
